CLINICAL AND RADIATION ONCOLOGY PARDUBICE

The Internal Regulations are binding for all patients of the Multiscan s.r.o. healthcare facility (hereinafter referred to as the healthcare facility) who receive specialized outpatient and dispensary care (hereinafter referred to as healthcare). It applies by analogy to visitors or escorts of patients.

The internal rules of the health facility are issued in accordance with Act No. 372/2011 Coll., on health services and conditions of their provision, as amended, in order to inform persons moving in the health facility about their rights and obligations and about the proper operation of the health facility.

Admission of patients to outpatient specialised and dispensary health care

- On admission to health care, the patient is obliged, at the request of a member of staff of the health care establishment, to prove his identity by means of an identity card, to produce a health insurance card and proof of temporary incapacity for work, if issued.

- If the patient is a foreigner/EU citizen with a valid European insurance card or a document replacing it, the patient must present these documents.

- The patient is provided with healthcare on the basis of a written informed consent, which is signed by the healthcare staff with the patient.

- The patient may withdraw his or her consent to the provision of healthcare. Withdrawal of consent shall not be effective if a medical procedure has already begun which, if interrupted, could cause serious damage to the patient's health or endanger his life.

- The patient has the right to express informed consent to the provision of healthcare. This shall be recorded in writing in the form of a negative reverse in the patient's medical record. Before signing the negative reversal, the patient shall be informed by the physician of the seriousness of the medical condition to the extent and in a manner that makes it clear that failure to provide health care may seriously harm the patient's health or endanger his or her life, and shall be informed of possible alternatives for treatment. All the information given to the patient is recorded on the negative reverse and placed in the patient's medical record with the signature of the informant, the informed and a witness (general nurse, nurse practitioner, radiological assistant or physician).

- As part of the informed consent, the patient shall indicate the names of the persons, the relationship of these persons to the patient and the contacts of the persons to whom information about the patient's medical condition may/may not be communicated, within a predetermined scope. These persons, as well as the scope of the information to be communicated, may be changed during the course of health care.

- In the case of a patient with limited legal capacity, the right to health information and the right to ask questions shall be vested in the patient's legal representative or guardian and in the patient if he or she is of reasonable mental capacity and freedom to do so.

- If the court assigns a guardian or legal deputy to the patient, everything outside the acute procedures must be discussed with these persons and informed consent is subject to their decision.

- In order to ensure the protection of personal and sensitive data, information about the patient's medical condition may only be disclosed to the persons named in the informed consent and upon communication of a predetermined password.

- When the patient is admitted to receive healthcare in a healthcare facility, he/she brings with him/her the required documentation (laboratory results, X-rays, treatment recommendations, pre-operative examinations, etc.).

- If the patient has any type of allergy, he/she is obliged to disclose this information to the healthcare facility staff.

Decision on temporary incapacity for work, on the need for treatment

The employed person is entitled to be issued with the following upon the decision of the attending physician:

- Decision on temporary incapacity for work. A decision on temporary incapacity for work is issued on admission for outpatient treatment, up to a maximum of 3 days in arrears, and on termination of incapacity for work 2 days in arrears. In cases where the patient's health condition does not allow for a statement on the issuance of temporary incapacity for work within the statutory period of 3 days, a request for retroactive recognition of incapacity for work may be submitted to the relevant department of the CSH.

- Decision on the need for treatment according to the legislation currently in force.

- Decision on the need for long-term health care according to the legislation currently in force.

Nursing staff

- All employees of the healthcare facility are visibly identified with a name tag indicating their name, surname, title and function.

- Teaching and training of medical students and medical school students takes place within the healthcare facility. Teachers and students are part of the medical team, participating in the diagnosis and treatment of patients under the supervision of fully qualified medical staff. The patient has the right to be informed of this fact and to refuse the presence of these persons at any time. This is recorded in the informed consent to healthcare (part of the patient's medical record).

Diagnostic and therapeutic plan

- Each patient is treated individually in the provision of healthcare on the basis of an individual treatment plan.

- The diagnostic and therapeutic plan is established by the physician in collaboration with the patient and other members of the healthcare team, based on the patient's current health problem and background, in a logical and timely sequence, on the basis of recommended medical and nursing procedures, supported by modern technology.

- The treatment procedure includes a diagnostic assessment, treatment design, treatment regimen. The patient is obliged to comply with the treatment regimen and truthfully inform the medical staff about the development of his/her health condition.

- The patient has the right to ask the attending physician for information about his/her state of health, the course of treatment, the results of individual examinations and the planned course of treatment.

- The patient shall be entitled to request the opinion of another physician under the conditions laid down by law.

- It is the duty of the medical staff to verify the patient's identity before each diagnostic and therapeutic procedure by asking for the patient's name, surname and date of birth. Before administering anticancer treatment, it is necessary to verify the data on the patient's identification bracelet or identification chip.

- Patients in the radiotherapy department shall enter the premises only after being invited to do so by the medical staff of the healthcare facility (controlled and monitored zone).

. When providing healthcare in the context of radiotherapy, the patient's escort must be instructed when entering the controlled zone and the stay in the controlled zone must be recorded in the controlled zone entry register, including the signature of the person entering the controlled zone. Pregnant women are not recommended to enter the controlled zone.

Patient's personal belongings

- On arrival at the outpatient clinic, the patient shall place his/her belongings in a designated area designated by the attending staff of the relevant department.

- The patient shall have a designated area for civilian personal clothing and footwear during examination/treatment/treatment.

- The use of mobile phones and other small electrical appliances shall be permitted only with the consent of the attending staff and provided that its use does not disturb other patients and healthcare workers in the performance of their duties.

- It is strictly forbidden to bring weapons, ammunition, long-bladed knives, explosives or other dangerous objects into the healthcare facility.

Drugs brought in

We do not administer or administer brought in medicines.

Regime measures

- The patient has the right to privacy.

- The patient is familiar with the operating procedures of the facility upon admission and is obliged to comply with them.

- At all times during healthcare, the patient is obliged to be respectful of other patients and their rights, not to interfere with the treatment regimen of other patients and to allow the healthcare staff to perform their duties.

- The patient shall behave courteously towards healthcare professionals and respect their instructions.

- The patient shall maintain cleanliness and order in all areas of the healthcare facility.

- The patient is obliged to be respectful of the equipment of the healthcare facility and not to destroy it. Financial compensation is required for any damage caused by the patient to the property of the healthcare facility.

- It is prohibited to take any photographs, video or audio recordings in all areas of the healthcare facility without the consent of the designated manager and without the consent of the persons who may be filmed or photographed.

- Failure to comply with the Internal Regulations may result in the premature termination of the patient's health care if the patient seriously restricts the rights of other patients, deliberately and consistently fails to comply with the proposed individual treatment procedure (if consented to), or fails to comply with the Internal Regulations while his/her behaviour is not due to a medical condition. Similarly, under the above provision, a health care facility may terminate a patient's health care when he or she ceases to provide the cooperation necessary for the continued provision of health care services. Termination of the provision of health services to a patient is regulated by the provisions of Section 48(2) of Act No 372/2011 Coll., on health services and conditions of their provision.

Familiarisation with the operation of the healthcare facility

- Every patient is obliged to familiarise himself/herself with selected documents of the facility (Internal Regulations, Fire Alarm Directive, Patients' Rights and Obligations) upon admission to outpatient specialised and dispensary healthcare.

- Fire alarm guidelines with marked escape routes are posted prominently on the walls of each health facility building.

- The Internal Regulations and the Rights and Duties of Patients of the health facility are freely accessible on the website www.multiscan.cz and available in written form at the reception desks of the health facility.

- The patient has the right to become familiar with the spatial layout of the healthcare facility where healthcare is provided.

- The opening hours are published on the entrance door of the healthcare facility and on the website of the healthcare facility www.multiscan.cz.

- A patient who has not made an appointment in advance (without prior telephone or e-mail arrangement with the health facility's nursing staff) may not be treated but will be given an alternative appointment.

- Due to the nature and operation of the facility, appointment times may be delayed.

Hygiene and epidemiological regime

- The patient is obliged to comply with the hygiene and epidemiological measures as instructed by the medical staff.

- During the stay in the healthcare facility, the patient must observe increased hand hygiene.

- Mechanical handwashing and hygienic hand disinfection shall be carried out in accordance with the instructions on the information boards.

- Patient escorts are prohibited from entering strictly aseptic areas of the healthcare facility (e.g. the intervention room).

Waste handling

- Waste is segregated in the healthcare facility according to the legislative requirements of the Czech Republic.

- In connection with patient care, the most common waste generated is municipal waste, hazardous waste, packaging material - plastics, glass.

- In the medical facility there are containers for municipal waste, which are marked "SMES".

- Plastic packaging is thrown out of the municipal waste into the designated containers, marked "PET BOTTLES ONLY".

- Glass waste is handed over by the patient to the unit staff.

Catering

In justified cases, the patient is given a consultation in the nutrition clinic.

Spiritual and spiritual services

In the provision of health care, the staff of the health facility respects the value system of the patients. The patient has the right to express his/her request for the provision of these needs at any time during the course of the health care provided. If there is a demand for these services, the health care staff will facilitate these services.

Alcohol and addictive substances

- Alcohol and other addictive substances are prohibited during the course of health care.

- The patient is obliged to undergo, at the physician's discretion, in justified cases, an examination to prove whether the patient is/is not under the influence of alcohol or other addictive substances.

- Smoking is prohibited in all buildings/areas of the medical facility and on the premises of the Hospital of the Pardubice Region, a.s., except in areas designated for smoking.

Opinions and comments

- Patients can give their opinions and comments on their stay in the healthcare facility by means of a satisfaction questionnaire designed for this purpose.

- Satisfaction questionnaires are located next to the boxes near the reception desk, at the entrance or can be requested from the medical staff.

- Completed questionnaires are dropped into the freely accessible boxes located near the building reception.

Expression of no/satisfaction

The patient has the right to express his/her satisfaction/dissatisfaction with his/her healthcare.

The patient may express his/her views verbally or in writing by means of a satisfaction questionnaire, which is located near the reception of the healthcare facility.

Complaints

- In the event of serious doubts about the course or quality of the healthcare services provided, the patient has the right to express his/her reservations.

- The form of complaint may be oral or written.

- The patient may contact any member of the healthcare team, who is obliged to inform the management of the facility immediately.

- The procedure for making a complaint is set out on the website www.multiscan.cz.

Presence of a dog with special training

- A patient with a sensory or physical disability who uses a guide or assistance dog has the right, in view of his/her current medical condition, to be accompanied and to have the dog present in the healthcare facility under the conditions set out below:

- the patient is able to provide routine care for the dog himself or herself or through another person,

- the presence of the dog is only possible with the agreement of the management of the establishment, taking into account all the circumstances, in particular the hygiene conditions and the rights of other patients,

- the patient/accompanying person must produce the dog's vaccination certificate with valid vaccinations required by law.

- The healthcare facility provides "canister therapy". This is a therapy that uses positive action on human health through a specially trained and led dog or bitch with its handler.

Discharge of a patient from outpatient specialised and dispensary health care

- The patient has the right to be informed of the planned discharge from health care.

- On discharge from health care, the patient shall be informed by the attending physician of the progress of treatment to date and the further planned course of action.

- The patient shall be provided with a medical report on discharge from health care.

- On discharge from health care, the patient shall visit his/her general physician and present the discharge report to him/her in order to consult on further treatment.

- After leaving health care, the patient is required to pay all charges for health services provided that are not covered by health insurance.

- Ambulance transport of patients who are unable to transport themselves to their place of residence or other designated place shall be decided by the attending physician upon discharge from health care.

Self-pay patient

- A patient has the right to be informed in advance of the cost of health services not or partially covered by public health insurance and the method of payment for them, if his/her health condition so permits.

- If a self-paying patient is admitted for planned health care, the referring physician is obliged to inform the Health Insurance Department of the health care facility of this fact, which, on the basis of the list of planned procedures, prepares a preliminary financial calculation for the self-paying patient for this care. The patient shall confirm his/her agreement with this preliminary financial calculation by signing the statement.

- The patient shall be obliged to reimburse the provider for the cost of health services not or partially covered by public health insurance or other sources which have been provided with his/her consent.

- The amount of payment for the provision of a health service shall be determined by the internal regulations of the health facility.

Social services

The services offered include social counselling, cooperation in the provision of social services, information service for people with disabilities.

Inspection of medical records kept on the patient, taking extracts and copies

- The medical records kept on the patient are the property of the health service provider.

- Access to the medical records, making extracts and copies is possible on the basis of a request to the quality department of the healthcare facility and payment of a fee according to the current "Price list for administrative tasks".

- The instructions for processing the application, the application form and the price list for administrative tasks are freely available for download on the healthcare facility's website www.multiscan.cz or on request from the healthcare staff or the Care Quality Department of the healthcare facility.

Previously expressed wishes

- In case the patient is in a state of health in which he/she is unable to consent or object to the provision of health services and the manner in which they are provided, the patient may express his/her consent or objection in advance.

- The previously expressed wish must be in writing, with a certified signature and written instructions on the consequences of such a decision by the registering general practitioner or other treating physician in the field of health care to which the previously expressed wish relates.

- A previously expressed wish may not be exercised in the case of a person with limited legal capacity. A previously expressed wish may not be respected if its fulfilment could endanger other persons.
